Connecting Consumers with Care

Overview

The Connecting Consumers with Care grant program area provides program support to Massachusetts community-based organizations, community health centers, and select hospital-based programs for services to help low-income consumers enroll in public and private health access programs, navigate the health care system, and connect with providers. The goal of this grant program is to strengthen community-based efforts that provide a continuum of services designed to ensure consumers understand, enroll and maintain coverage under MassHealth, Commonwealth Care or other health access programs, and utilize that coverage to access health care services to which they are entitled.

Program Status

Twenty-three grantees were selected for a one-year grant of $20,000-25,000 by the Foundation's Board of Directors in December 2009.  The next opportunity to apply for this funding will be in the fall of 2010. Guidelines for the 2010 grant cycle will be available in the summer of 2010.
